Sickle Cell Institute Chhattisgarh (SCIC), Raipur .

Adult haemoglobin is a globular protein molecule made up of two pairs of polypeptide chains designated as globin "α" and globin 'β', each folded around a heme molecule. The function of haemoglobin molecule in red blood corpuscles (RBCs) is to carry oxygen to different tissues and disorders in its function can lead to severe abnormalities. It was Herrick, who for the first time in 1910 described 'peculiar elongated and sickle shaped RBCs in a case of severe anaemia, which was later on termed as sickle cell anaemia.

In sickle cell anaemia, the abnormality occurs in the β chain of globin molecule i.e. - in 6th position of the peptide chain glutamic acid is replaced by valine. The sickle cell hemoglobinopathies are of two major categories, viz., sickle cell disease and sickle cell trait. Sickle cell trait acts as a carrier for sickle cell disease. The hemoglobin is represented as HbSS (Homozygous state) in sickle cell disease and as HbAS (Heterozygous state) in sickle cell trait. Sickle cell anaemia was characterized as a genetic disease affecting the synthesis of hemoglobin. Under conditions with lack of oxygen, the RBCs in the blood of patients suffering from this disease assume the shape of sickle, hence the name.

In India, the sickle cell gene is distributed mainly in Madhya Pradesh, Chhattisgarh, Maharashtra, Orissa, Jharkhand, parts of Andhra Pradesh, Kerala, Karnataka and Tamil Nadu. In some communities, the prevalence of the sickle cell trait is as high as 30% of the total population.

Individual suffering from sickle cell disease (HbSS) usually complain of repeated generalized body pain, anaemia, jaundice, repeated blood transfusion, pneumonia, repeated infection etc. Their lifespan is usually reduced to 5-25 years. Hence, they require specific attention and treatment and care.

Executive Summary of Sickle Cell Institute Chhattisgarh in Raipur

• So far the screening work for detection of sickle cell disease and carrier state has revealed that about ten percent (10%) of the population is suffering from this genetic disorder.

• The proposed project is "Establishment of Sickle Cell Institute in Raipur". The Institute will be a dedicated centre for treatment, research, counseling and training related to sickle cell disease.

• Requirement of land for the establishment of the institute, is approximately 25 acres in the capital city of Raipur.

• The institute will have a completely dedicated 100 bedded hospital for sickle cell disease patients which will be equipped with OPD, casualty, wards, intensive care unit (ICU), operation theatre (OT), blood transfusion facilities, diagnostics lab etc.

• Similarly a there will be a research division dedicated to sickle cell disease research, which will be equipped with cytogenetics, molecular genetics, gene silencing& stem cell, applied medical genetics, medicine & cancer research, Genetics& intervention, bioinformatics& biostatistics, and isotope & animal house laboratory facilities.

• Other amenities will incluse training centre, hostels, guest house, maintenance division and other miscellaneous requirements.

• The institute will be run on 100 percent financial aid from Govt. of Chhattisgarh. Budget Requirement for the establishment of infrastructure for Sickle Cell Institute will be about Rs. 60 crores for the period of 4 years. This will comprise the nonrecurring expenditure as Rs. 10 crore, 14 crore, 25 crore, and 11 crore in 1st, 2nd, 3rd and 4th year respectively. Approximately Rs. 10 crores will be the recurring expenditure every year after including salaries, consumables, medicines, chemicals, maintenance and other expenses.

• The expected outcome of the project will be to provide exclusive facilities for :
A. Diagnosis, treatment, counseling of the sickle cell patients.
B. Creating public awareness for this genetic disorder, to provide training to health care personnel of the state for diagnosis and treatment of the disease.
C. Advanced research on various aspects of this disease.
It will increase confidence among sufferer of the genetic disorder.

• This institute will function as totally autonomous institute.

• There will be a Governing Council headed by honorable Health Minister of the state. The members will be the Additional Chief Secretary of the state, Vice Chancellor of the Ayush and Health Sciences University of Chhattisgarh, Principal Secretary Health, Principal Secretary Law, Commissioner Health, two outside members viz., renowned scientists of the country, Director Medical Education, Director Health Services, Director AYUSH, and collector Raipur. Member Secretary will be the Director General of the Sickle Cell Institute Chhattisgarh. All the policy decisions will be taken by this committee. The committee will meet at least once in a year and if needed emergency meeting may be called with the permission of the chairman.
